Co/Extracurricular Participation Forms
Students in grades 9-12 who wish to participate in a sport or club are expected to attend a code meeting and submit the required form(s) before participation may begin.
Juniors - must turn in the Activities & Code Signature Page by October 1 to be eligible to be voted prom king or queen
Seniors - must turn in the Activities & Code Signature Page by October 1 to be eligible to attend the senior trip
Medical Eligibility Form or Alternate Year Athletic Permit Form - call 764-5431 ext. 1148 to find out which form you need
Code Form - code video is available (see below). No in-person meeting this year. The video is mandatory. Please watch it along with a parent/guardian.
